Eyelid surgery or "blepharoplasty" is a surgical procedure to remove loose skin and other tissue which cause the skin to hang over the eyelid or eyebrows.

What is blepharoplasty?

Eyelid surgery is a surgical procedure to remove loose skin and other tissue which cause the skin to hang over the eyelid or eyebrows. Due to its weight, the excessive skin makes the eyes heavy which is distinctive especially when the eyes are tired and at late hours when it can even close the visual field.

The procedure thus has its aesthetic as well as functional aspects since the loose skin can narrow the visual field. The
loose skin can be accompanied by the ptosis or eyelid drooping which means that due to different reasons, the levator muscle is not able to sufficiently lift the eyelid.

For a successful procedure an eyebrow lift is also required along with the loose skin removal. Without it, the forehead skin would be additionally pulled down and the condition could be repeated in a few months due to it.

What are the benefits of blepharoplasty?

The eye area is the first thing we notice in another person. At the same time, due to thin skin, it is also the first thing that starts to show signs of aging on our face. Eyelid correction is a permanent and effective procedure that achieves a neater, rejuvenated and natural appearance.

We can remove:

- skin that causes excess wrinkles on the upper and lower eyelids,
- swelling,

These conditions can be the result of natural aging or a genetic predisposition. The problem of excess skin is common in patients over 30-40 years of age, while eyelid swelling is more common in younger patients. If you have severely wrinkled skin on the upper eyelids, which makes it difficult to see in the corners of the field of vision, surgery will correct this. Of course, it cannot cure other vision problems. There are no special age restrictions, but the procedure is not recommended if you have serious eye diseases.

Is the result permanent after the procedure?

We cannot "freeze" your face from further aging, but the effects of the procedure are very long-lasting. Reoperations are usually not necessary and the skin looks much better than if the procedure had not been performed. It is important that you have realistic expectations about your post-operative appearance and that you follow all the instructions we will give you during your recovery.

How is eyelid correction performed?

Basically, we distinguish between upper and lower blepharoplasty. They are performed on both eyelids (e.g. upper blepharoplasty of both eyelids, even if one is very droopy and the other minimally droopy) to ensure the harmony of the face. We can also decide to perform both procedures at the same time, i.e. upper and lower blepharoplasty, which is called correction of all four eyelids.

Upper eyelid correction or upper blepharoplasty

Upper blepharoplasty is performed under local anesthesia and takes about an hour. The excess skin of the upper eyelids is removed surgically.

Skin removal can be accompanied by other procedures (removal of excess fat of the upper eyelids, correction of the position of the eyebrows, correction of ptosis (sagging) of the upper eyelids).

Excess skin on the upper eyelids is a result of aging. The skin hangs over the upper eyelid or even over its edge, giving the feeling of heavy eyelids, and can also reduce the field of vision. As a result, upper blepharoplasty is both an aesthetic and a functional procedure. In addition, with aging, the fat bulge of the upper eyelids can also appear, which is removed through the same incision, and the drooping of the eyebrows, which can also be corrected to some extent through the same incision.

The surgical incision is hidden in the crease of the upper eyelids and becomes almost invisible after a few months. When the incision is extended beyond the outer edge of the eye due to excess skin, the scar may be visible for a long time. The stitches are removed after one week. Bruises last up to about 10 days, minor swelling up to three months. At that time, the final result of the operation is also visible.

Correction of the upper eyelids is often performed in combination with facial rejuvenation with fillers (forehead lift), face and neck lift (facelift) or brow lift (eyebrow lift). Drooping eyelids can originate from drooping eyebrows or forehead.

Lower eyelid correction or lower blepharoplasty

Lower blepharoplasty is usually performed under general anesthesia. Most of the time, we focus on removing or redistributing fat bags under the eyes. The incision is made just below the lower lashes and will be minimally visible with the correct technique.

Lower blepharoplasty includes several different surgical interventions, which are performed depending on the problems we want to eliminate, namely:

- TRANSCONJUNCTIVAL LOWER BLEPHAROPLASTY is a procedure where, with the help of an incision through the conjunctiva (inner side of the eyelid), excess fat of the lower eyelid is removed, which may be a result of aging or may appear genetically.
This is why we perform such an intervention on both the elderly and younger people. The procedure can also be combined with the removal of excess skin through an external incision under the eyelashes and the strengthening of the lower eyelid (canthoplasty or canthopexy).

- CLASSIC LOWER BLEPHAROPLASTY is a procedure performed through an external incision just below the eyelashes of the lower eyelid. In this procedure, we can also remove excess fatty tissue, which can also be moved downwards and used to fill the groove under the lower eyelid. The procedure is associated with several complications.

- LIFTING OF THE MIDDLE PART OF THE FACE is a continuation of the classic lower blepharoplasty. Through the same incision, the tissues of the cheeks are released and they are pulled up and fixed with stitches.

- All procedures can also be combined with the injection of your own fat. During aging, fatty tissue gradually disappears in the middle part of the face, which is replaced by our own fat.

- CANTHOPEXY and CANTHOPLASTY are surgical procedures that can be performed independently or as part of lower blepharoplasty. With them, we interrupt and shorten the lower eyelid (canthoplasty) or just tighten the lower eyelid (canthopexy), thus preventing or eliminating ectropion or scleral show. These are conditions where the skin of the eyelids and cheeks pulls down the edge of the eyelid and deforms the eyelid.

The surgical incision (if it is external) takes place just below the edge of the lower eyelid and becomes practically invisible in a few months. Occasionally, it also continues beyond the line of the edge of the outer corner of the eye, where it can remain minimally noticeable for a long time. The outer stitches are removed after one week. Bruises persist for up to 10 days, minor swelling up to three months (up to half a year in the case of lifting). At that time, the final result of the operation is also visible.

Upper and lower eyelid surgery at the same time

In certain cases, it is best to perform upper and lower blepharoplasty at the same time (correction of all four eyelids). This is how we achieve the maximum effect. The result is raised upper eyelids and the simultaneous elimination of dark bags under the eyes, which allows for a fresh and rested appearance. Even in this case, we operate on you under general anesthesia.

How is the recovery after eyelid correction?

Band-aids are placed over the incisions. You are discharged to home care on the same day. It is normal to experience swelling, bruising, irritation or dry eyes after surgery. They are caused by the sensitivity of the skin and the adaptation of the eye to the change. Discomfort is controlled with medication, cold compresses and ointments. We will give you detailed instructions on recovery.

After the procedure, it is recommended that you avoid straining your eyes and looking at electronic screens for about a week. We also advise you to take time off from work during this period. After a week, you will return to our office for a follow-up examination.

You can start using contact lenses and makeup two weeks after surgery.

What are the possible complications and risks?

As with any surgery, we must be aware of possible complications. These are rare in experienced surgeons. They include infection, bleeding, and improper healing or scarring. They are additionally avoided by following the doctor's instructions on recovery. Dryness or excessive tearing of the eyes, sensitivity to light, or other vision changes may also occur. The latter complications are usually transient. This means that they disappear during recovery, when the eye gets used to the change in the surrounding tissue.

Are there alternative treatments for droopy eyelids?

Alternative procedures to surgical removal of the skin of the upper eyelids do not exist. Minimal improvements are possible with the help of outpatient procedures that tighten the skin of the eyelids (plasmapen, laser, peels).

Results

The results of blepharoplasty begin to show when the swelling and bruising subsides, which is approximately 10 to 14 days after surgery. Final results are usually visible after a few months and are long-lasting in most cases. Scars also fade during this time.
